Nose Bleeding/ Epistaxis

This is nothing but a haemorrhage or bleeding from the nose.

Epistaxis may be the result of traumatism. It may be the precursor of certain infectious diseases as typhoid. It may be due to weakness of the blood vessels of the nose. In elderly people, it may be due to changes in the walls of the blood vessels. It is frequent in cirrhosis of the kidney and liver. It may be due to ulceration from diseases or from picking the nose.  It may be due to diseases like anaemia and chlorosis. In women, it sometimes replaces the regular menstrual flow.

Morbid anatomy & pathology: Infrequent recurring epistaxis the nose must be examined to see if any structural change has taken place.

Symptoms: The symptomatology is simple. It consists of bleeding. Most cases bleed from the septum side of the anterior nares. Occasionally it is far back. In that case, or if the bleeding occurs at night, the blood may get into the nasopharynx and be coughed out or swallowed and vomited.

Complication and Sequelae: Occasionally the haemorrhage or bleeding is so profuse that it causes anaemia.

Treatment: In the majority of cases of simple bleeding cold water applied to the nose will stop it, or cold at the back of the neck – traditional “Key down the back” – or holding the hands above the head etc.

Pressing the nostrils on the affected side will stop it usually. The pressure should be kept up till the clot forms, and care must be exercised not to blow the nose till the clot is formed.

Symptoms may be following after anger, bathing, blowing nose, during chill, with convulsions, with cough, during dinner, in diphtheria, during fever, during/after headache, with haemorrhoids or piles,

The symptoms are usually seen in children but they can also be seen in the elderly, especially in young women with irregular menses. Bleeding may occur during washing the face or feet.
